Dear Local 455 Kroger Members of the Atlanta Health & Welfare Fund,
In order to enroll dependents for coverage, the Fund must receive certain documents to establish that each dependent qualifies as an “Eligible Dependent,” as defined by the Fund. If you have received a Documents Request from the Fund Office, you must submit the required requested documents before April 30, 2025 in order to complete the dependent verification process.
Please note that some forms are due annually, every 3 years, or only once. If you have submitted the requested documentation in years’ past, you may still need to submit the documents again for 2025 coverage.
If the Fund does not receive the required documents by April 30, 2025, your dependents will be terminated from coverage, and you will have to appeal to the Board of Trustees to attempt to get them reinstated. Please do not wait any longer. Please cooperate with the Fund’s request immediately.
